Many serious accidents have occurred as a result of a machine started up when it was supposed to shut off for maintenance or repairs. These accidents are usually caused by stored energy that is not properly released. Lock-out-tag-out (LOTO) that ensures that machines are off until a person with authorization releases them, helps to prevent these incidents.
The LOTO system is comprised of a set of locks tags, locks, and other equipment that are used to isolate, secure or block the machine from electrical sources. All "affected" and "authorized" personnel must be educated on the use of these lockout/tag-out devices. These devices include but aren't only chains, locks padslocks, wedges adapter pins and self-locking fasteners. Each device is clearly identified and labeled with the date and the time of application along with the name of the authorized person, and the reason for the tag-out or lockout (repair or maintenance, for example). It must be clear that these devices are not able to be removed until a thorough LOTO procedure has been followed.
